Work Permit Medical Certificate
Chiang Mai RAM Medical Center
Work Permit Medical Certificate
UPDATED 12 February 2020
I needed to get the Work Permit Medical Certificate so I could apply for my work permit. Here’s the latest information about getting the Work Permit Medical Certificate in Chiang Mai.
Getting the Chiang Mai Work Permit Medical Certificate has just gotten a lot easier. I just went down to the new Medical Centre at the RAM Hospital on the west side of the moat, behind the Starbucks.
In 2020, walked in at 8:50am, drove out at 9:20am, then popped back to pick up the certificate at 12pm.
In 2017, walked in about 12pm, walked out 15 minutes later (see below).

Registration
Firstly you take your license or passport, and get registered at the registration desk. After that you, have a quick height, weight and blood pressure check, then go and give blood and within a couple of minutes you’re seen by Dr who actually gives you some prodding, asks some questions and listens your heart beating and chest function and then you walk out.
There are these beautiful waiting areas but never really even sat for a minute.

Cashier
Of course before you leave you have to pay your 373 baht fee, then pop back at your leisure after two hours and pick up the medical certificate and report.

I used to think the RAM Hospital was expensive, but it’s only a few baht more than my normal hospital (220 baht, but normally at least 90 minutes or 2 trips across town), and less than many others. I suggest going here to get your yearly work permit medical certificate.

Frequently Asked Questions
What is a Work Permit Medical Certificate in Thailand?
A medical certificate from a Thai-registered doctor confirming you are free from specific prohibited conditions required for work permit applications. The standard form covers leprosy, tuberculosis, elephantiasis, drug addiction, third-stage syphilis, and several other conditions. Most Thai clinics and hospitals can issue this.
Where can I get a Work Permit Medical Certificate in Chiang Mai?
Any Thai-registered clinic or hospital. The certificate requires a doctor's signature and official stamp. Private clinics near the immigration office on Airport Road provide this service quickly, often same-day. The cost is typically 300-500 THB.
How long is a Work Permit Medical Certificate valid?
Typically 30 days from the date of issue. Use it promptly. The Labour Department and Department of Employment will check the date. If your certificate is older than 30 days at the time of submission, you will need a new one.
Does a Work Permit Medical Certificate require blood tests?
For the standard work permit application, the medical examination is typically a basic physical check by the doctor rather than lab tests. The doctor verifies you do not show clinical signs of the listed conditions. For certain work categories or visa types, additional tests may be required. Confirm with your employer or the issuing authority what is needed.
Can I use a medical certificate from my home country?
No. Thai work permit applications require a medical certificate from a Thai-registered doctor. The certificate must be in the specified Thai government format or an accepted equivalent. A translated foreign certificate is not a substitute.
Guru Tip
Get the medical certificate on the same day as your Residency Certificate from Immigration. The clinics near the Airport Road Immigration office are familiar with the work permit medical form and process it quickly. Call ahead to confirm the clinic has the correct form and knows the work permit format before making the trip. Some small clinics only issue basic health certificates and not the specific government form required.
